Medical Form

Parents will need to submit a physical & immunization form. Please take a moment to check that your child’s physical will be valid. For example: the physical must be stamped within the calendar year. If you do not have a form you may click above to print and have your child’s doctor complete it.

Note: : You may register your child without this form but all children must have a physical submitted before starting. Physicals can be submitted on ProCare at any time using the “ documents “ tab.
